NDIS Hospital to
Home Transition

What is hospital-to-home transition under NDIS?

NDIS hospital-to-home transition is a coordinated discharge service where a registered provider manages clinical handover, accommodation readiness, support worker allocation, and post-discharge stabilisation to safely move a participant from acute care back to the community — preventing readmission.

Our team liaises directly with hospital discharge planners and treating clinicians to coordinate SIL placements, ensuring every participant lands somewhere safe, supported, and stable. The pace of the process depends on the participant's complexity and available compatible homes — we are honest about timelines from the first conversation.

In-Home SIL
Support

What is In-Home SIL Support?

In-Home SIL Support is Supported Independent Living delivered in a structured family-home setting — a real house, in a real neighbourhood, with professional support built around the rhythms of ordinary home life. It differs from a group home in feel: smaller, quieter, and more personal.

For some participants, living in a group home doesn't feel right — the noise, the roster of unfamiliar faces, or the institutional quality that can creep into larger settings. In-Home SIL aims to replicate the experience of a family home, where the people who live there know each other, the house has its own character, and support workers become familiar presences rather than interchangeable shift staff.

Key Features

Real home, real life

A genuine residential property — not a purpose-built care facility. Chosen for its neighbourhood, access to transport, and proximity to things the participant cares about.

Consistent support team

Smaller rostering pool means participants are supported by familiar faces — building genuine relationships rather than managing an ever-changing roster.

Goal-focused planning

Support plans built around what matters to the participant — cooking, community, work, creativity — not just task completion.

NDIS registered & compliant

Full NDIS registration with clinical oversight, incident reporting, and quality safeguarding — none of the paperwork burden falls on the participant.
